The Department of Student Support Services is here to provide students, parents, and campuses with guidance in the following areas: federal, special education, and early childhood programs, counseling, pregnancy-related, speech, and homebound services, medical and mental health issues, crisis intervention, homelessness, occupational and physical therapy, and Section 504.
Senate Bill 12 (Parental Consent)
During the 89th Texas Legislature, Senate Bill 12 was passed, which requires annual parental consent for students to receive school health-related services, including routine medical and counseling services as provided by trained staff. In compliance with this new law, qualified school staff, including nurses, other health professionals, athletic trainers, counselors, mental health staff, and administrators, must have written consent on file before providing any school health-related services to students. In the event of a serious emergency, staff will take immediate action to protect your child’s safety and contact you.
Annual Routine Health Services Consent Form
Senate Bill 12 allows for school districts to request parental consent for routine health services, like first aid or crisis response, on a once-annual basis at enrollment and/or at the beginning of the school year. This will allow Galena Park ISD to continue taking care of students’ routine health needs.
To meet this requirement, an Annual Routine Health Services Consent Form has been added to your child’s Skyward account. Please log in to your child’s Skyward Family Access account to review and complete the consent form. This link will take you to the Skyward Family Access System Skyward Family Access Link. We kindly ask that parents/guardians complete this form no later than October 1, 2025.
If this form is not completed by October 1, 2025, school staff will be unable to provide your child with routine physical or mental health services, such as basic first aid or wellness checks, unless the care is required to provide life-saving care to a child in an emergency.
If any health services are needed beyond the routine services included on the Annual Routine Health Services Consent Form, campus staff will contact the parent to discuss their child’s additional needs and seek further consent for non-routine health services recommended.
New students to the district can now complete the form via the registration process.

Viewing Consent Status - Parents can view their consent status for Physical Health Services and Mental Health Services in Skyward Family Access.
Update Consent - To update your response, visit Skyward Family Access. Select the Annual Routine Health Services Consent Form and then resubmit the form with new responses.
