If you are interested in enrolling your child(ren) in the Galena Park ISD Childcare Center, please complete the form below for the 2024/2025 school year.  Upon receipt of the form, the center will contact you to confirm availability and the expected start date.

Step II - Download and Complete Forms

Please download and complete the applicable forms below after your child has been accepted for enrollment for the 2024/2025 school year.  Forms are due one week before your child's first day of attendance.

• • Required forms to be completed by parent:  2024/2025 Parent Enrollment Forms
  • Required forms to be completed by child's physician: 2024/2025 Required Medical Forms
  • Optional forms to be completed by child's physician if applicable: Optional Doctor's Form

Step III - Submit Forms to Center

You may submit your documents one of two ways:

1.  Email the completed and signed documents to your selected center's clerk or director.

2.  Bring the completed and signed documents to the center no later than one week before your child's first date of attendance.  Please call before coming to ensure a staff member is available to receive your forms.
