Parental Assumptions

  • An adult assuming responsibility for a minor living separate and apart from a parent, legal guardian, or other person having lawful control under a court order will have to complete an annual Parental Assumption.  Parental Assumptions are available electronically by emailing student_admissions@galenaparkisd.com.

    The Parental Assumption guidelines are as follows:

    1. The legal parents/guardians of the student are:
      1. Deceased (must provide death certificate)
      2. Incarcerated (must provide System Personal Number)
      3. Living/working out of state (proof of employment)
      4. Whereabouts unknown

    A NOTARIZED POWER OF ATTORNEY IS REQUIRED FOR ALL PARENTAL ASSUMPTIONS.

    Attach the following documentation to the online application:

    • Birth Certificate (required for new Parental Assumptions)
    • Current Utility Bill (required)
    • Parent/Guardian Driver's License or Other Issued ID
    • Notarized Power of Attorney 
    • Previous School Records (required for new Parental Assumptions)

    Students applying for enrollment under a Parental Assumption will not be enrolled until all requirements have been met and approval is granted.
